Social Worker

Job Description

Reporting to the Assistant Manager, you will provide psychosocial support to the palliative patients (who are taken care of by our Home Hospice team).


Key Responsibilities

  • Provide and facilitate psychosocial assessments to help cancer patients and their families manage impact of illness and progression
  • Provide on-going individual and family counselling through face-to-face sessions or home visitations in order to help our beneficiaries navigate personal and ethical dilemmas, cultural conflicts and issues relating to religion, spirituality and the meaning of life with the impact of cancer
  • Support patients and their families at the end-of-life stage through counselling or programmes to support meaningful living
  • Provide bereavement information, education and support to help the families of the deceased patients cope with their grief and loss.
  • Prepare social and other related reports for management and other related institutions / agencies
  • Attend trans-disciplinary team meetings with medical staff, nurses, and other allied healthcare professionals to discuss, propose and coordinate care plans for patients
  • Liaise with other health care professions and external agencies to coordinate care for patients / families


Job Requirements

Technical Competencies:

  • Case management
  • Bereavement counselling

 

Core Competencies:

  • Strong interpersonal skills
  • Able to connect with different individuals from diverse cultural background
  • Good communication and written skills

 

Preferred achievements / characteristics:

  • You must have a degree in Social Work and are comfortable working with older persons in the home hospice care environment
  • Able to speak a second language such as a Chinese dialect or Malay
